What is
mdPersonal
TM? It
is a new kind of medical care in which I am able to
provide the personalized, on-time care that I believe
benefits you most. We all know that preventive
medicine and early detection save lives. By limiting my
practice to no more than 600 patients, rather than the
two to four thousand patients most doctors now care for,
I have the time to provide comprehensive preventive
care to each one of you who becomes a member. Time to
focus on long-term solutions to your health-care
problems and time to educate you and research your
specific concerns and needs. Time to develop
individualized wellness plans, to keep you as healthy as
possible for as long as possible.
What is the
annual fee? The current annual fee is $2040
per individual, $3640 per couple, $3840 for family of
three, $4040 for family of four, $2040 for single parent
and one child. For families greater than four members,
it is $480 for each additional child. If you

If I sign up, do
I still need medical insurance? Yes, you
still need insurance.
mdPersonal
TM membership is not a substitute for
medical insurance. The annual fee is for services,
including the Annual Comprehensive Wellness Examination,
that are not considered covered benefits by insurance
plans or Medicare. If any of the listed services are
considered covered benefits, then the annual fee is for
the remaining list of services. For a list of services
included in the annual fee, please read further down on
this page.
I have Medicare,
can I still join? Yes, you can still join
mdPersonal
TM
provided you sign the application and agreement form and
pay the annual fee. Nothing else will change, except
that you will get a free annual comprehensive wellness
examination and all the benefits of membership listed
below. The annual fee covers services that are not
included in Medicare's list of covered services.
I have HMO
insurance, CAN I STILL JOIN? At
mdPersonal
TM
we do not contract with any HMO's.
You can still become a member and receive all the
benefits listed below (24/7 phone and email access,
etc.), but your actual visits to me will not be covered
under your HMO insurance. You will be responsible for
the cost of all office visits (excluding the annual
comprehensive wellness examination, which is covered in
the annual fee) and labs that are not done under your
HMO physician. If you decide to join
mdPersonal
TM
we will be more than happy to offer
a significant cash discount for office visits and
medical care. You will still need to sign up with
another PCP and can then use your HMO insurance as a
form of hospitalization coverage for major medical
expenses. However, if you want to receive insurance
coverage for your office visits, you will have to go
through your HMO physician for all specialty referrals,
lab and x-rays in order to get coverage.
What services
and benefits are included in the annual membership fee?
24/7 direct phone and e-mail access to me; same-day
or next-day appointments that are on-time and can be for
as long as needed; no waiting time or minimal waiting
time in a comfortable private office setting; house
calls for you and your family when you are too ill or
unable to make it to the office; detailed coordination
of specialty care, testing and accompaniment to
specialists when appropriate; annual comprehensive
wellness examination and my continued commitment to
personalized care, emphasizing wellness and disease
prevention.
Will you
bill my medical insurance? Yes, your office
visits will continue to be conveniently billed by our
office staff.

What are my
payment options? You can pay by personal
check or credit card. Special financial needs will be taken into
account. Please write to Dr. Neimark for consideration.
Is the annual fee tax
deductible? If your company has a Flex Plan
(Section 125) or a Medical IRA, you may be able to
receive reimbursement for your annual payment with
before tax dollars. I encourage you to talk with your
tax advisor and employer about these options.
If I need to be hospitalized, where
will I go and who will take care of me?
I am proud to be affiliated
with Hoag Memorial Hospital Presbyterian in Newport
Beach, rated one the best hospital in Orange County and
among the top 100 hospitals nationwide for its high
quality patient care and extensive specialty services. I
use the services of Hoag Hospital's exclusive 24 hour
Hospitalist Group to manage your care while
hospitalized. I will work closely with these physicians
to assure all your in-patient and discharge-planning
needs are being met.
What should I do if I have an
emergency on the weekend or after normal business hours?
I will be
available for consultation 24 hours a day, seven days a
week. However, there may be times when you require an
emergency room for care. I highly recommend the expert
staff of Hoag Hospital's Emergency Department, or the
Hoag-affiliated urgent care clinics to aide in you care.
I will always help to make sure that you are well taken
care of.
Since I need to keep my insurance,
will there be any problem with reimbursement of outside
tests ordered through you?
No. I am a participating physician with most PPO plans.
Depending on your individual policy, most plans will
cover medically necessary tests as long as they are
performed at a facility contracted with you insurance.
My office will make every attempt to direct you to the
appropriate facility for the best coverage.
What happens if I move after I
join? If you must
move and transfer care, I will assist you with finding a
new physician. Your medical records will be sent
promptly upon your signed release of information. The
balance of your yearly fee will be prorated and refunded
with a 90 days written notice. |
