ANNUAL ENROLLMENT FEES

• $240 per month per individual
  
• $400 per month per couple
  
• $480 per month for family of three (2 adults, 1 child < 18 y.o.)
  
• $540 per month for family of four (2 adults, 2 children < 18 y.o.)
  

The cost for additional children < 18 y.o. is $80 per month for each child.
