ANNUAL ENROLLMENT FEES

• $240 per month per individual
  
• $400 per month per couple
  
• $480 per month for family of three
  
• $520 per month for family of four
  
• $260 per month for single parent and one child (<18yo)

For families larger than four, it is $60 per month for each additional child.
