Scheduling & Visits

What should I expect at my initial consultation?

During your initial consultation, Dr. Neimark will carefully review and discuss your medical history as well as perform any needed physical exams. If you are a good candidate for our services, Dr. Neimark will discuss the necessary lab tests and schedule your follow-up appointments.

What's the cost for the initial consultation?

The initial consultation is US$397.

What Are The Restrictions On Virtual Doctor's Visits?

If you are NOT an in-state resident, your initial visit at mdPersonal Concierge Functional Medicine must be in person. After the first visit, all follow up visits can be conducted by phone or virtually using our secure video channel if you choose.

Email Etiquette

Patients have unlimited email access with our mdPersonal Concierge Functional Medicine provider team. However, if an email question or request is deemed by one of our staff to be too complicated or involved for a response via email they may, at their discretion, require that the patient schedule a phone, telemedicine or in person visit instead of responding via email.

Scheduling & Cancellations

There is a 24-hour cancellation policy for all appointments with an mdPersonal Concierge Functional Medicine doctor, health coach, or team member. Canceling within 24-hours will result in a $50 charge to the credit card on file.

No-shows: If you do not show up to a phone, video/telemedicine or in person appointment without notifying the specific person you have an appointment with either via email or by changing the appointment using the online scheduling tool on the patient portal, you will be charged $100 to the card on file.

Billing & Insurance

We Are Not A Conventional Primary Care Physician

For many of our patients, we can take the place of a primary care physician. Because you can message us online during business hours, we are often able to take care of acute requests. For example, prescribing an antibiotic for a minor infection such as a sinus infection.

It is important to note the following however:

  • We do not fulfill an insurance company’s requirement for an in network primary care physician as we are not in-network with any insurance plan.
  • We will make every effort to make you fully aware of any additional lab fees that may be incurred and to give you the opportunity to choose which lab tests you feel comfortable paying for.
  • We do not have a 24-hour answering service and do not take calls on weekends. If you need to be seen urgently or have a medical emergency you would need to visit an ER or urgent care center.

We Do Not Bill Insurance Companies

Any services provided by mdPersonal Concierge Functional Medicine that aren’t already included in the program fee will be billed to patients directly.

mdPersonal Concierge Functional Medicine medical expenses, such as lab tests and prescription medications, may be covered by a patient’s health insurance plan (per that plan’s rules).

mdPersonal Concierge Functional Medicine will not submit a medical claim to insurance on your behalf. We cannot assist you with claim resolution for our services.

We will provide you with a detailed billing summary that you may submit to your insurance for reimbursement. There may be procedures or lab tests that are not covered depending on your plan’s rules.

About Lab Tests

  • Most lab tests ordered by a mdPersonal Concierge Functional Medicine physician will be billed directly to your insurance by the lab.
  • Some lab tests we offer as part of our functional medicine testing program may not be covered by insurance and in these cases you will be charged a fee for a particular test by the lab or through mdPersonal Concierge Functional Medicine.
  • The specific tests ordered will be personalized to you based on your initial evaluation with an mdPersonal Concierge Functional Medicine doctor and may include blood saliva urine or stool testing.
  • We will make every effort to make you fully aware of any additional lab fees that may be incurred and to give you the opportunity to choose which lab tests you feel comfortable paying for.
  • You are under no obligation to purchase any specialty testing.
  • Should any lab results indicate you may need further testing you will be directed to continue your care with your primary care physician or a relevant specialist, or should you choose you may continue to work with your mdPersonal Concierge Functional Medicine physician.
  • At home test kits: If a lab test kit is mailed to you by mdPersonal Concierge Functional Medicine, you will be charged the day the kit is mailed.
  • If your particular insurance company only pays for blood work done at a particular lab facility like LabCorp, you must inform us of this in advance of having blood work done. Most insurance plans cover labs done by major national labs – however, including Bioreference, which is a lab often used by mdPersonal Concierge Functional Medicine because of their favorable pricing policies for patients.

Scheduling & Visits

During your initial consultation, Dr. Neimark will carefully review and discuss your medical history and get to better understand your health goals. He will then discuss how our signature mdPersonal Health Roadmap will be specifically applied to your unique situation. If he thinks there is a different doctor or center of excellence that may be a better fit for you, he will recommend the best next step. If both you and Dr Neimark feel you are a good candidate for our services, then we will discuss the costs and benefits of our concierge membership, and if you are interested in becoming a member, we will have you fill out the appropriate membership forms and start the process of booking your first appointment, to get started on your journey towards greater health and healing. 
The initial 30 minute consultation usually runs $249 but is being offered to new patients at a discounted rate of $149.

Patients have unlimited email access with our mdPersonal Concierge Functional Medicine provider team. However, if an email question or request is deemed by one of our staff to be too complicated or involved for a response via email they may, at their discretion, require that the patient schedule a phone, telemedicine or in person visit instead of responding via email.
As a general courtesy, we request any appointment cancellation be made as soon as possible before your scheduled appointment, so we can accommodate another patient at that time slot. Though there is no charge for a missed appointment or no-show, we may contact your mother to find out why you skipped etiquette classes as a child!

Billing & Insurance

For most of our patients who have Medicare or a private health insurance, we can and do act as their primary care physician. We are not primary care providers for any HMO, managed care or Medicaid plans. Feel free to call us during regular office hours, so we can get you the help you need in a timely manner. We only take 

It is important to note the following however:

  • We do accept most private insurance and Medicare insurance plans, but you need to check with our patient care coordinator to make sure we are on your plan.
  • We are not providers for any HMO, Medicaid or managed care programs. 
  • We will make every effort to make you fully aware of any additional lab fees that may be incurred and to give you the opportunity to choose which lab tests you feel comfortable paying for.
  • We are available 24/7 for all emergency calls and once you become a member you will receive Dr Neimark's personal cell phone number for any after hour emergencies.
Your mdPersonal Concierge Membership includes access to our office staff for general advice and guidance, but all medical appointments with the doctor or nurse practitioner will be billed to your health insurance provider. You are responsible for any deductible, copays or coinsurance you may have. 

Other expenses such as outside lab tests, x-ray services and prescription medications, may be covered by your health insurance plan (per that plan’s rules). However, there may be procedures or lab tests that are not covered depending on your plan’s rules. We will always try to inform you about any tests that are not typically covered by insurance. 

mdPersonal Concierge Functional Medicine will happily submit medical claims to your insurance plan on your behalf. We utilize an outside billing service that can assist you with any claim resolution issues for our provided services.
  • Most lab tests ordered by an mdPersonal Concierge Functional Medicine physician, nurse practitioner or PA, will be billed directly to your insurance by the lab.
  • Some lab tests we offer as part of our functional medicine testing program may not be covered by insurance and in these cases you will be charged a fee for a particular test by the lab or through mdPersonal Concierge Functional Medicine.
  • The specific tests ordered will be personalized to you based on your initial evaluation with an mdPersonal Concierge Functional Medicine doctor and may include blood saliva urine or stool testing.
  • We will make every effort to make you fully aware of any additional lab fees that may be incurred and to give you the opportunity to choose which lab tests you feel comfortable paying for.
  • You are under no obligation to purchase any specialty testing.
  • Should any lab results indicate you may need further testing, we will advise you on the recommended next steps.
  • At home test kits: We utilize a company called Rupa Health to manage most functional medicine testing involving at home lab kits. Many of these tests include the cost of the test and a small processing fee added on by Rupa Health and/or mdPersonal Concierge Functional Medicine, that are your responsibility to pay.
  • If your particular insurance company only pays for blood work done at a particular lab facility like LabCorp, you must inform us of this in advance of having blood work done. Most all of our routine blood tests are performed at Quest Labs. Most insurance plans cover labs done by major national labs.
